List of Supporting Documents

Beginning Fall 2022, to speed up an applicant’s review process, all supporting documents must be submitted as a packet.  Packets must be complete upon submission – incomplete packets will not be accepted.

Determining eligibility relies heavily on supporting documents.  To efficiently review an applicant’s eligibility for financial assistance, he/she must carefully select the appropriate application type (NEW, ONGOING, or RETURNING) to effectively determine the types of documents needed for the review.  Selecting the wrong application type and Program Type will result in a delay in the processing of any awards.

*Please note your PACKET will not be reviewed without an application.

*The Online Application deadline is Friday - July 1st, 2022.

*The deadline for submission of your CSO PACKET is Friday -September 30, 2022.
